We look forward to seeing you at this year’s Symposium! In order to comply with COVID-19 best practices and safety we have secured Crowdpass to assist us with review of proof of vaccination status or negative test result.
- Proof of full COVID-19 vaccination indicating at least 14 days since your second Pfizer or Moderna shot, or since your single Johnson & Johnson shot.
- Proof of a COVID-19 negative test must indicate that the test was taken less than 72 hours before the event. The COVID-19 test should be a negative laboratory-based test. We are accepting both PCR and Rapid Antigen tests.
- When presenting your proof of a negative test, it must include the type of test, the test date, and the name of the person being tested.
- It is required that proof of a negative test result be uploaded before the event. In order for Crowdpass to approve your registration, you must upload proof of vaccination status.
- Have you or people whom you have close contact with, like someone who lives in your house, a relative, a friend, or a co-worker tested positive for COVID-19 in the last 14 days? Have you had any COVID-19 symptoms or have been given an order to quarantine?
- If yes, have you followed CDC quarantine guidelines below:
- If you were exposed to COVID-19 and not up to date on COVID-19 vaccines, stay home and quarantine for at least 5 full days. After 5 days, watch for symptoms and wear a well-fitting mask and do not travel, until 10 days after last had close contact with someone with COVID-19.
- If you were exposed to COVID-19 and up to date on your COVID-19 vaccine, you do not need to quarantine, but wear a well-fitting mask until 10 full days after exposure.

After you upload your information and are approved, you will receive an email confirming your approval. It will contain a QC code that you will need onsite to check-in prior to receiving your registration badge. Be on the lookout for an additional reminder to come.
